Pre-information form
This form is given before you accept an offer, under the Regulation on Distance Contracts. Together with the details of the offer you are about to accept, it informs you of the essential terms of the contract.

The service
Coordination and organisation of the treatment and related services described in the offer: the treatment by the named clinic or doctor, and — where the offer includes them — accommodation, transfers, interpreting and similar services. The medical service is provided by the provider named in the offer.
Total price and additional costs
The total price is written in the offer, in its currency, taxes included. Costs not included in the offer — for example flights, unless stated — are listed as excluded. The offer is valid until the date written in it.
Payment
Bank transfer, EFT or SWIFT to the account in the payment instructions, within the stated period and with the payment reference; a deposit, if foreseen, as written in the offer. Card details are never requested.
Performance
On the dates in the offer and in the reservation plan you confirm; the provider confirms the treatment after its medical assessment.
Right of withdrawal
You may withdraw within 14 days of the contract's formation without giving a reason, by notifying [email protected] or using the portal; amounts paid are refunded within 14 days, without cost to you. The right does not apply to services whose performance has started with your approval within that period, or to accommodation and similar services for a specific date; cancellation then follows the cancellation and refund terms and the offer's own cancellation terms.
Complaints and disputes
Write to [email protected] with any complaint. Within the yearly monetary limits you may apply to the consumer arbitration committee, and above them to the consumer court, of your place of residence or of the transaction.
