Medical intake

Definition

Medical intake is the step before treatment in which the patient shares their complaint, medical history, photos and documents, and a doctor assesses whether they are suitable and what the plan might be.

What does an intake ask for?

A patient enquiring about a hair transplant is asked for photos from the front, top and sides; someone asking about implants for photos of their teeth or, if they have one, a panoramic X-ray. Questions about long-term conditions, medication, allergies, smoking and previous procedures come alongside. With this the doctor judges whether the patient looks suitable for treatment and whether a provisional plan is possible.

Building the form around the patient's language and the treatment saves days lost to missing photos. Showing example pictures of how to take each photo, and keeping the questions to what is needed, helps more patients finish the form.

Pre-assessment tab: the dental questionnaire Oliver Hartley filled in on the patient portal, and Dr. Can Örnek's review: suitable for All-on-4.
A case's intake tab: the form sent and returned through a secure link, with the doctor's review below.Real product screen · fictional sample data
